Your first visit to Dr. Patricia Schultz establishes a key part in our relationship with you. Upon entry, we obtain the relevant background information and medical history; if you’d prefer to have this ready in advance, you can print the new patient registration form online, complete it and bring it with you to your first appointment.

Insurance and Billing Policies
We accept a variety of insurance plans; verify with your insurance provider that Dr. Schultz participates with your current insurance. Also, please verify with your insurance provider whether your individual policy will cover your foot care. We provide electronic billing for participating coverage.

All managed care patients who present for care without appropriate referrals and authorizations will not be seen. These insurers prohibit care without proper documentation. Participation in an insurance plan does not guarantee that services will be covered or approved or paid. All patients are personally responsible for timely payment for all services. Any services unpaid by your insurance company after 60 days will be the responsibility of the patient.
Co-payments, deductibles and payments for non-covered services and supplies must be paid at the time of service. We accept personal checks with identification. Credit and debit cards are also accepted.
Every patient is allotted the time needed to address their issues.
Appointment Policies
Our goals are timely care and fair scheduling for all patients. Therefore, require a minimum of one business day’s notice in advance of the appointment time for any changes of appointments or cancellations. Please be advised that a $25.00 missed appointment fee will be charged to you and must be paid before the following visit.
Cell Phone Use
While technology has changed dramatically, good medicine has not. Patients should refrain from using mobile phones once treatment has begun. Brief, quiet and unobtrusive calls may be allowed before treatment time.
