New Patients, Directions & Registration Forms

Your first visit to Dr. Patricia Schultz establishes a key part in our relationship with you. Upon entry, we obtain the relevant background information and medical history; if you’d prefer to have this ready in advance, you can print the new patient registration form online, complete it and bring it with you to your first appointment.

 

Below you will also find a map and directions to our office, practice hours and payment policies. We invite you to take some time to review our web site, and familiarize yourself with your symptoms. Being well prepared for your visit will ensure the doctor has the necessary information to make an assessment and provide you with the best possible care.

Insurance and Billing Policies

We accept a variety of insurance plans; verify with your insurance provider that Dr. Schultz participates with your current insurance. Also, please verify with your insurance provider whether your individual policy will cover your foot care. We provide electronic billing for participating coverage.

All managed care patients who present for care without appropriate referrals and authorizations will not be seen. These insurers prohibit care without proper documentation. Participation in an insurance plan does not guarantee that services will be covered or approved or paid. All patients are personally responsible for timely payment for all services. Any services unpaid by your insurance company after 60 days will be the responsibility of the patient.

 

Co-payments, deductibles and payments for non-covered services and supplies must be paid at the time of service. We accept personal checks with identification. Credit and debit cards are also accepted.

 

Every patient is allotted the time needed to address their issues.

 

Appointment Policies

Our goals are timely care and fair scheduling for all patients. Therefore, require a minimum of one business day’s notice in advance of the appointment time for any changes of appointments or cancellations. Please be advised that a $25.00 missed appointment fee will be charged to you and must be paid before the following visit.

 

Cell Phone Use

While technology has changed dramatically, good medicine has not. Patients should refrain from using mobile phones once treatment has begun. Brief, quiet and unobtrusive calls may be allowed before treatment time.

By Car

From Washington, DC by Georgia Avenue

Drive north on Georgia Avenue.  After crossing Eastern Avenue, Colesville Road is 8 stoplights, 1.4 miles.  Turn right on Colesville Road.  Proceed 3  stoplights, then 3 long blocks in the left lane.  The office is located at the corner of Mansion Drive and Colesville Road, on the left side.  Turn left–the office parking lot is to your immediate right.

From Washington, DC by 16th Street
(or Bethesda/East-West Hwy)

Drive north on 16th Street.  From the large traffic circle at the Maryland line, bear right onto Colesville Road.  Proceed 5 stoplights and 3 long blocks in the left lane.  The office is located at the corner of Mansion Drive and Colesville Road, on the left side.  Turn left–the office parking lot is to your immediate right.

From the Capital Beltway (Exit 30B)

Proceed south 3 stoplights on Colesville Road and three blocks after passing Dale Drive in the right lane.  The office is located at the corner of Mansion Drive and Colesville Road, on the right side.  Turn right –the office parking lot is to your immediate right.

By Public Transit

The office is 3/4 miles north of the Silver Spring Metro Station.  From Silver Spring Station, there are numerous “Z” Metrobuses and multiple Ride-On buses.  Exit at the Spring Street stop and cross Colesville Road.  Walk north 3 long blocks. The office is located at the corner of Mansion Drive and Colesville Road.
